2. Response to Electronic Communication

S2M Health will make its best effort to provide a timely response to electronic inquiries. However, please refrain from submitting emergency situations requiring immediate attention electronically. 

Moreover, our ability to respond to electronic communications depends on the information provided. Insufficient information may hinder our ability to offer accurate and reliable services. 

3. E-mail Privacy 

Notification messages regarding information managed by S2M Health may be sent to your e-mail address. Only an authorized person with access to this e-mail account should view these notifications. Please be mindful when reviewing e-mail messages or sending them. 

4. Security and Confidentiality  

We extend the same level of confidentiality to medical and proprietary information stored on S2M Health systems as we do to information in any other medium. Our commitment to protecting this information includes limiting employee access based on their role, utilizing firewalls, passwords, encryption, and audit trails. We log records accessed, noting the time and date of access for each entry.  

5. Privacy Notice 

This privacy notice discloses the practices of S2M Health. It applies solely to information collected by S2MHealth.com and covers:  

• What personally identifiable information is collected, how it is used, and with whom it may be shared. 
• Choices available to you regarding the use of your data. 
• Security procedures in place to protect against misuse of your information. 
• How you can correct any inaccuracies in the information. 

6. Electronic Health Information  

In accordance with the ONC Cures Act final rule on Information Blocking, effective 4/5/2021, S2M Health responds to queries for electronic health information (EHI) for in-home assessments and telehealth visits, complying with HIPAA privacy rule standards.

7. Information Collection, Use, and Sharing

S2M Health is the sole owner of information collected on this site. We only access/collect information voluntarily given via email or direct contact. We will not sell or rent this information and will use it to respond to your inquiries. 

8. Your Access to and Control Over Information

You may opt out of future contacts from S2M Health at any time. Contact us via the provided email address or phone number to: 

• See what data we have about you.
• Change/correct any data we have about you.
• Have us delete any data we have about you. 
• Express concerns about our use of your data. 

9. Portal License Agreement 

S2M Health provides a user portal (the “Portal”) for individuals (the “Users”) assigned by the Corporation to access information provided by S2M Health (the “S2M Health Information”). Use of the Portal is subject to guidelines, Terms of Use, and the Privacy Policy. The Portal is deemed S2M Health's intellectual property.

S2M Health grants the Corporation a non-exclusive, non-transferrable, limited license to use the Portal solely for accessing the S2M Health Information. Corporation is responsible for User security and promptly notifying S2M Health of any User termination or unauthorized disclosure.
