Our Process
Training
Project-specific training will be conducted for the coding team, and screening will be done. Those who clear the test will be part of the new project.
Coding
The team is committed to following all ICD 10CM, HCC, and project-specific guidelines to capture the valid codes for the scope of the year.
Audit Process:
All coded charts will be audited to ensure the delivery of charts with a minimum of 97% quality.
Calibration
Calibration is the process of gaging the accuracy between our auditors and the client. Few of the processes we use are GR&R and brainstorming on guidelines.
Rebuttal Process:
The audit log will be reviewed by the coders along with auditors daily to identify and address repeated errors in the future. The rebuttal comments will be validated by SMEs, and the required feedback will be shared with the coder/auditor.
RCA and Action Plan Process:
All consolidated audit logs will be reviewed by the SMEs, who will share error insights and action plans to address them. Training sessions will be conducted for conceptual errors. Users who have not met the quality for more than two weeks will be moved to the training team.
Random/Focused and Master Audit:
Random, focused, and master audits will be done to ensure the quality of both coded and audited charts, apart from the 100% audit.
